The path in brief
Graduates of medical schools outside the United States and Canada generally need ECFMG certification before entering U.S. residency (GME). Certification includes passing USMLE Step 1 and Step 2 CK, meeting the clinical and communication skills requirement, and having your medical education credentials verified. Your school must be listed in the World Directory of Medical Schools with an ECFMG note.
Requirements and fees change, so always confirm the current rules directly with ECFMG and the USMLE program before you register.
Common challenges for IMGs
- Different curricula. Your school may have taught topics in a different order or depth than U.S. schools. Practice by subject to find gaps quickly.
- Question style. Step 1 uses long clinical vignettes that test mechanisms. Regular practice with board-style questions builds reading speed and pattern recognition.
- Units and lab values. The exam uses U.S. conventions. Get used to looking up reference ranges the way you will on test day.
- Cost. Exam fees, certification, and travel add up. Free study resources can reduce the burden.
